The term ‘chronic disease’ can be off-putting. What it means is a long term health condition that needs ongoing treatment and management.

The CDM Programme aims to improve the health and well being of patients living with specific chronic diseases:

  • Type 2 Diabetes
  • Asthma
  • COPD
  • Cardiovascular Disease (including heart failure, heart attack or angina, stroke and irregular heartbeat (atrial fibrillation))

It is open to patients over the age of 18 who have a Medical Care or Doctor Visit Card.

Approximately 1 million people in Ireland are living with one of these conditions.

What Does the Programme Include?

The programme includes 2 free reviews in every 12 month period that you are part of the programme. Each review includes one visit with the Practice Nurse and a follow up visit with your GP. Our practice nurse will do a general health check including measurements such as weight and blood pressure. She will perform blood tests and a heart tracing (ECG) if warranted.

At your follow up appointment with your GP we will go through your measurements and advise you if they are in a healthy range. We will discuss you blood test results. A medication review is very important to ensure you are on optimal treatment. We will agree on a care plan going forward incorporating lifestyle, medication and any referrals that may be required.
